CERTIFICATE OF NEED NOTICE

Purcell SNF Operations, LLC applied to the Oklahoma State Department of Health (Department) for approval to acquire by operations transfer agreement and lease Purcell Care Center. Purcell Care Center is a one hundred five (105) bed nursing home facility located at 801 North 6th Street, Purcell, OK 73080. Purcell SNF Operations, LLC is the proposed licensee/operator/lessee. Purcell SNF Operations, LLC is the proposed licensee/operator/lessee. Tide Health Group, LLC is the sole member of Purcell SNF Operations, LLC, and Brian Ramos is the manager and person with controlling interest. The annual lease cost is four hundred twenty thousand dollars ($420,000.00) and the lease length is fifteen (15) years.

The application was filed under the state certificate of need laws and may be viewed electronically through Box by submitting a request to healthresources@health.ok.gov. Anyone desiring to participate in the review process must file a written notice of intent to participate, and file written evidence or argument. Notices of intent to participate must be received by the Department no later than April 29, 2026. Send notices to Health Resources Development Service, 123 Robert S.
